Findings: Alleged Victim (AV) moved into the facility in November 2019. At time of admittance, Alleged Perpetrator 2's (AP2) job responsibility was to put AV's INR blood draw into the facility's Medication Administration Record (MAR). AP2 did not put the INR blood draw into AV's MAR which resulted in AV not getting his/her INR blood draw until January 2020. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to have a safe medication administration system which placed AV at potential risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
