Findings: The facility failed to ensure supervision and implement meaningful and effective interventions in place regarding Witness #1 (W1) and his/her many altercations with residents. On or about January 13, 2020, W1 pushed the Alleged Victim (AV) causing him/her to fall to the ground, causing an injury to his/her arm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
