Findings: According to documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan to provide two-person assistance with transfers. The failure resulted in AV falling and breaking his/her Tibia, AP2's actions are considered neglect of care which constituents abuse. The facility failed to protect the AV from neglect, which is a violation of Oregon Administrative Rules.
