Findings: Alleged Victim (AV) was a fall risk. On or about January 14, 2020, at approximately 6:30 am AV was found on the floor of his/her room. AV complained of chest pains and was sent to the hospital. On January 14, 2020, at approximately 4:20pm, AV was found on the floor with a skin tear to his/her forearm and right knee. The facility failed to implement interventions to mitigate AV's risk of falls after the first fall, which is a violation of resident rights, is neglect of care and constitutes abuse.
