Findings: Facility staff transferred the Alleged Victim (AV) inappropriately by using his/her hands to pull him/her up while transferring, causing bruising to his/her hands. Staff did not have the appropriate training to understand how to transfer the resident properly to avoid pain and/or bruising to the resident. The facility's failure to provide a safe environment and properly train staff, placing the resident at risk for harm is a violation of resident rights, is considered neglect of care and constitutes abuse.
