Findings: Alleged Victim (AV) and Witness 4 (W4) have a known history of altercations. On or about January 19, 2020, AV and W4 got into an altercation resulting in AV receiving a large bruise on his/her arm. On or about January 21, 2020, AV and W4 had four separate altercations resulting in AV receiving a skin tear and being hit. On or about January 22, 2020, AV and W1 had a verbal altercation. The facility failed to properly care plan related to AV and W4's history, which is a violation of resident rights, is neglect of care and constitutes abuse.
