- Findings
- On or about January 18, 2020, Alleged Perpetrator 2 (AP2) did not follow Alleged Victim's (AV) care plan when he/she assisted AV with a shower. While AP2 assisted AV with showering, AV became combative with AP2. AP2 responded to AV's combative actions by slapping AV on the wrist and by spraying water on AV's face. AV's actions are considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
