Findings: The facility failed to follow the Alleged Victim's (AV) care plan to complete safety checks every 30 minutes due to fall risks. On or about January 13, 2020, AV had an unwitnessed fall, was found in his/her room with dried blood on his/her head. The fall resulted in a laceration to his/her head. AV was transferred to the hospital where he/she received 5 staples in his/her head. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
Sanction: RCFCP21-01684 $1500.00 fine assessed
