Findings: On or about January 10, 2020, Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) failed to properly transfer the Alleged Victim (AV) using a gait belt, causing an injury to his/her ankle and bruising to his/her torso. AP2 and AP3's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failed to protect the AV from harm by failing to update AV's care plan to include use of a Hoyer lift rather than a gait belt. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
