Findings: Alleged Victim (AV) relies on the facility to provide housekeeping services. On or about January 19, 2020, AV was found lying on the floor of his/her bathroom. AV stated he/she was cleaning the shower mat which was dirty with mildew and slipped on water that was on the floor. AV suffered a fractured arm due to the fall. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00275 $375.00 fine assessed
