Findings: The facility failed to provide a safe medication administration system for Alleged Victim's (AV) pain patch. On or about January 15, 2020, AV experienced an increase in pain and unreasonable discomfort due to a medication error when staff failed to remove the patch from AV the day before. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
