Findings: On or about January 25, 2020, Alleged Victim (AV) was observed with having a bruise under his/her left eye. An investigation determined facility could not explain how the bruise occurred and facility did not have documentation indicating AV's bruise was investigated by the facility for an injury of unknown cause. The facility failed to provide a safe environment for AV which is a violation of resident rights, constitutes abuse, and is considered neglect of care.
