Findings: The facility failed to appropriately care plan regarding Alleged Victim's (AV) risk for falls to ensure AV's safety. An investigation determined that AV had approximately fourteen (14) documented falls between December 26, 2019 and January 25, 2020. As a result of AV's fall on January 25, 2020, AV suffered a fracture to his/her collar bone. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
