Findings: The facility failed to implement appropriate interventions and staff support regarding known fall risks related to the Alleged Victim (AV). AV has had multiple falls, of which, some staff were not aware of. On or about January 27, 2020, AV fell and sustained a laceration to his/her forehead. Despite AV's falls, no new interventions were put in place for staff to know how to reduce the risk of falls for this resident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
