Findings: On or about January 25, 2020, the Alleged Victim (AV) and Witness #1 (W1) engaged in a physical altercation. W1 is known to have altercations and many interventions were in place, however, on January 23, 2020, W1 was was removed from the high risk list at the facility due to behaviors improving. On January 25, 2020, W1 shook the hand of AV, then got upset and hit AV and pulled AV's hair. There were no major injuries other than redness to AV. The facility failed to provide a safe environment, which placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02174 $250.00 fine assessed
