Findings: The facility failed to assess, intervene, and monitor when Alleged Victim (AV) experienced a change of condition. An investigation determined that AV lost 31.8 pounds over a 13 month time period. The facility failed to address AV's rapid weight loss and to ensure that AV's dietary guidelines/modifications were followed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
