Findings: The facility failed to provide a safe medication administration system and ensure the Alleged Victim's medication was administered as ordered. The failure resulted in AV being administered twice the dosage of his/her medication for approximately 12 days exposing him/her to risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
