Findings: The facility failed to intervene and timely seek medical treatment when the Alleged Victim (AV) experienced a significant change of condition, from multiple falls with injury and a significant reduction in weight. The AV was transported to the hospital and was diagnosed with a fractured hip and malnutrition. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
