Findings: The facility failed to implement interventions and appropriately care plan according to the Alleged Victim's (AV) prior fall after returning from the hospital with a hip fracture. The failure resulted in AV experiencing two more injury falls and was transported back to the hospital for treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
