Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. The AV had multiple unwitnessed falls from mid December until January 29, 2020, causing a range of injuries from bruising to a broken toe. The facility's failure to provide a safe environment and to properly care plan to avoid falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
