Findings: The facility failed to ensure supervision and staff support regarding a known behavior of the Alleged Victim (AV) to leave the facility without assistance. In the two months prior to this elopement, the facility was aware that AV had a change of condition, including decreased cognitive ability. On or about February 1, 2020, AV eloped from the facility and was found across the street in a driveway sitting on his/her walker, placing AV at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
