Findings: Alleged Victim (AV) resides in a secured facility and has been identified as an elopement risk. AV has a history of waiting by exiting doors to attempt to elope. On or about February 4, 2020, a visitor notified staff that AV was outside in the parking lot. It is presumed that AV exited by following the visitors out as the visitors left the building. Staff went outside and got AV to come back into the building unharmed. The facility failed to provide a safe environment putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
