Findings: The facility failed to follow Witness #1 and the Alleged Victim’s care plan to monitor and redirect when the residents were near each other to prevent altercations. On or about January 31, 2020, the two residents had an altercation in front of Witness #1's room. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
