Findings: On or about February 1, 2020, Alleged Perpetrator #2 (AP2) spoke inappropriately to the Alleged Victim while he/she was in the dining room, causing a loss of dignity, undue stress and emotional abuse. AP2's actions caused a loss of dignity, stress and emotional abuse to the Alleged Victim, which is a violation of residents rights, is considered neglect of care and constitutes abuse. The facility failed to protect the resident from emotional abuse, which is a violation of Oregon Administrative Rules. On the same day, it is alleged that AP2 wrongfully restrained the Alleged Victim, however, an investigation inconclusively determined no wrongdoing or abuse occurred.
