Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) and Witness #1 (W1) regarding their risks of altercations. W1 has a history of wandering into other residents’ rooms, however, was not care planned to be redirected and AV had a history of aggressive behaviors when others entered his/her room. On or about February 9, 2020, both residents were found on the floor after an altercation and AV was later found to have a broken wrist, requiring medical attention. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
