Findings: The facility failed to ensure supervision and staff support to ensure the Alleged Victim was safe. On or about October 30,2019, the Alleged Victim went into the kitchen area and sprayed disinfectant into his/her mouth. AV did not have any adverse side effects, however, this action did place AV at a risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
