Findings: On or about December 15, 2019, Witness 1 (W1) was noted to be anxious upon waking up and while walking down the hall, he/she became upset with Alleged Victim (AV) and pushed AV down the hallway. The facility failed to appropriately care plan and implement interventions according to W1's known behaviors and history of altercations. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
