Findings: The Alleged Perpetrator 2 (AP2) failed to appropriately monitor and provide the Alleged Victim (AV) with necessary services after he/she had been found after a fall. The failure resulted in AV experiencing a second fall and was on the floor for an undetermined amount of time and later transported to the hospital for treatment. AP2 is responsible for neglect of care which constitutes abuse. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
