- Findings
- Alleged Victim (AV) is care planned to be checked every two hours to ensure his/her needs are being met and is care planned for assistance with incontinence care and general housekeeping. On or about February 14, 2020, AV was checked on around 8:00pm. Alleged Perpetrator 3 (AP3) did not perform another check until approximately 4:20am the next morning, in which AV requested assistance with changing soiled clothing and bedding. At approximately 4:40am, AP3 was asked to assist AV by a med tech. AP3's shift was from 10:00pm, February 14th through 6:00am February 15, 2020. Per Video of AV's room AP3 did not provide cares as needed to AV. Alleged Perpetrator 2 (AP2) started his/her shift at 6:00am on February 15, 2020. AP3 communicated to AP2 that AV needed assistance with care. Per video of AV's room AP2 went into AV's room several times between the hours of 7:00am and 9:40am, each time AV requested assistance. At approximately 10:20 am AV received assistance with care needs by a family member. AP2 and AP3 did not follow AV's care plan resulting in AV experience unreasonable discomfort which is considered neglect of care and constitutes abuse. The facility failed to ensure AV's care plan was followed which is a violation of Oregon Administrative Rules.
