Findings: The facility failed to provide a safe medication administration system for Alleged Victim's (AV) by failing to provide oversight and supervision resulting in AV's pain medication being diverted and/or administered too frequently, placing AV at risk of serious harm. The allegation that Alleged Perpetrator 2 (AP2) financially exploited AV was investigated and findings were inconclusive. An investigation determined that AP2 and Alleged Perpetrator 3 (AP3) over-administered AV's pain medication, placing AV at risk of serious harm. AP2's and AP3's actions are considered neglect of care and constitutes abuse.
