Findings: Alleged Victim (AV) is known to become any when other residents touch his/her walker or gets to close to AV. Witness 1 (W1) is known to be easily irritated and argumentative. W1 and AV had a prior altercation on or about December 25, 2019. On or about February 20, 2020, AV and W1 were in the dining hall. W1 tried to take AV's walker. W1 punched AV in the forehead causing AV physical harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
