Findings: On or about February 24, 2020, Alleged Perpetrator 2 (AP2) administered the wrong medication to Alleged Victim (AV) which was not part of the facility service plan agreement. AP2's actions is considered neglect of care which constitutes abuse. The facility is responsible for the supervision, training, and overall conduct of staff when staff are acting within the scope of his or her job duties. The facility's failure is a violation of Oregon Administrative Rules.
