Findings: The facility failed to follow the Alleged Victim's care plan to be a one person assist with a walker and gait belt during ambulation. On or about February 23, 2020, the Alleged Victim fell while ambulating alone, was transported to the hospital and diagnosed with a cervical vertebrae fracture. The facility 's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
