Findings: The facility failed to provide appropriate supervision to the Alleged Victim (AV). On or about February 25, 2020, Witness 1 (W1) was involved in a precursory event which escalated to a physical altercation between AV and W1. The failure resulted in W1 hitting AV on his/her head and AV falling to the ground and experiencing distress. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
