Findings: The facility failed to appropriately care plan for Alleged Victim (AV) regarding his/her risk of falls. On or about February 24, 2020, AV fell and was injured with a hip fracture that required surgery. An investigation determined that facility staff and facility administration were aware that AV had an unsteady gait that resulted in previous falls. Facility staff were not provided with training or direction on how to provide interventions to prevent AV from falling due to AV's unsteadiness and cognitive impairments when ambulating. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
