Findings: Alleged Victim (AV) moved into the facility on or about February 12, 2020. AV is care planned to have bathing assistance and incontinence care. AV did not receive a shower until February 25, 2020. AV experienced long wait times for incontinence care. The facility failed to provide bathing assistance and timely incontinence care resulting in AV experiencing increased anxiety and skin breakdown. The facility failure is a violation of resident rights, is neglect of care and constitutes abuse.
