Findings: Alleged Victim (AV) had a known history of falls and had interventions in place to mitigate AV's risk of injuries. On or about February 2, 2020, AV was identified as needing a new placement due to increase needs/care. AV experienced falls on February 6th, 7th, 8th, 9th, and 10th, 2020. The facility failed to care plan and implement interventions related to AV's increase of needs/care and increased falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
