Findings: The facility failed to provide a safe environment for Alleged Victim (AV). The facility neglected AV by failing to put interventions in place to address and prevent resident-to-resident altercations between AV and Witness 1 (W1) after an altercation on February 2, 2020, which resulted in another altercation on February 15, 2020 in which AV sustained an injury to his/her head. The facility's failure is a violation of resident rights, is considered neglect of care, and constitutes abuse.
