Findings: On or about February 11, 2020, Alleged Victim (AV) suffered from a fall and was transported to the hospital for treatment. An investigation determined that AV had documented falls prior to date of incident. Facility documentation shows AV fell on January 3, 2020, February 2, 2020, and February 11, 2020. The facility failed to appropriately care plan for Alleged Victim (AV) fall risk and failed to put proper interventions in place to prevent future falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00579 $1500.00 fine assessed
