Findings: On or about February 26, 2020, Alleged Victim (AV) had an unwitnessed fall that resulted in AV being diagnosed with a left clavicle fracture and bruising on his/her left chest area, forearms, and left thumb. Facility implemented a temporary service plan after the incident; however, AV suffered from additional unwitnessed falls after AV's temporary service plan was put into place. An investigation determined that the facility failed to follow AV's care plan which a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00938 $375.00 fine assessed
