Findings: The facility failed to appropriately care plan for Alleged Victim (AV) regarding his/her risk of falls. An investigation determined that AV suffered from six (6) falls between December 27, 2019 and January 23, 2020 and the facility did not update AV's care plan to address AV's fall risk and failed to put interventions in place to prevent future falls until after AV's 6th fall. The facility's failure to properly plan care for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
