Findings: On or about February 26, 2020, Alleged Victim (AV) received assistance from staff with a transfer which resulted in AV almost falling. An investigation determined that Alleged Perpetrator 2 (AP2) failed to follow AV's care plan which put AV at potential risk of harm. AP2's actions is considered neglect of care and constitutes abuse. The facility is responsible for the supervision, training, and overall conduct of staff when staff are acting within the scope of his/her employment duties. The facility failed to ensure staff followed AV's care plan which is a violation of resident rights, is considered neglect of care and constitutes abuse.
