Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). AV has had multiple bruises of unknown origin over a period of time. The facility has not put into place any new interventions to try to reduce the number of bruises that the AV gets. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
