Findings: The facility failed to follow the Alleged Victim's care plan to have a baby monitor on in his/her room for safety. On or about February 23, 2020, AV left his/her room without anyone being aware, and he/she fell and broke his/her nose and received many bruises to his/her face. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
