Findings: On or about March 1, 2020, Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan to utilize a two person transfer while toileting AV. During the transfer, AV cut his/her toe on the tile of the bathroom. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide proper training to staff, causing a risk of harm to residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00483 $375.00 fine assessed
