Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). On or about January 15, 2020, the AV and Witness #1 (W1) had an altercation where W1 pulled AV's hair, causing unnecessary distress to AV. The facility's failure to ensure resident safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
