Findings: The facility failed to enter the Alleged Victim's (AV) medications into the Medication Administration Record (MAR) when AV moved into the facility. The facility also failed to follow up on AV's medication needs with AV's physician to ensure AV was getting the medication necessary. Due to this error, AV experienced a decline in behavioral issues, placing AV and staff at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
