Findings: According to documentation, Alleged Perpetrator 2 failed to protect Alleged Victim (AV) from physical abuse when s/he used physical force against AV when providing cares. The failure resulted in AV experiencing a bruise to his/her wrist and AP2's actions are considered physical abuse. The facility failed to protect AV from abuse which is a violation of Oregon Administrative Rules.
