Findings: Alleged Victim (AV) had a known history of falls. On or about March 7, 2020, AV suffered a fall in which he/she sustained a head injury as well as a fractured humerus. The facility failed to care plan and implement appropriate interventions to mitigate AV's risk of injury due to falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
