Findings: The facility failed to implement effective interventions and appropriately care plan according to the Alleged Victim's (AV) known fall history. The failure resulted AV experiencing approximately fifteen falls in about three months, the last of which causing AV to be transported to the hospital with a broken clavicle, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
